Health Provider Database Page

Hello! I use this page/database to keep track of most of my medical stuff, and as someone with a boatload of chronic illnesses, it’s been incredibly helpful. Each entry is for a provider/group of providers. There’s spots in each entry to put what health system they’re affiliated with, your last appointment with them, your next appointment, their contact info, their specialty (pulmonology, dentistry, cardiology, general surgeon, etc), their status, and their frequency. This page is set up so that you can see at a glance who you need to schedule your next appointment with/when your next appointment with them is. There’s also a template for each page with a spot for notes for your appointments. In my personal version of this, there’s also a relation to my task database, so I can link tasks that relate to providers (getting lab tests done, etc). If you use notion calendar, make sure to make it so that this database is visible on it! Also, this page is way better in dark mode.
